At FedPoint, we are looking for a Team Leader Care Coordinator Clinical Support to join our dynamic Care Coordination Department. About the Role:

As the Team Leader, you will be responsible for overseeing a team focused on providing exceptional service to claimants and healthcare professionals. Your leadership will guide your team in navigating clinical referrals, ensuring quality training, and delivering top-notch care coordination.
Key Responsibilities:

  1. Supervision and Team Development (55% of your time)
    • Lead, motivate, and coach Care Coordination Associates to enhance their skills and performance.
    • Provide guidance on program explanations and assist claimants and their representatives in maximizing their insurance benefits.
    • Manage clinical referrals and prepare monthly scorecards with quality results, statistics, and audit verifications.
    • Conduct frequent team meetings to provide feedback and communicate key information.
    • Deliver monthly performance evaluations and collaborate with team members to set individual objectives aligned with departmental goals.
    • Assist in the hiring and training of new staff to ensure a high-performing team.
  2. Quality Assurance and Training (30%)
    • Oversee the performance and development of the Care Coordination Clinical Quality Lead and Care Coordinator Clinical Trainer.
    • Ensure the maintenance of policies, procedures, workflows, and training manuals for the Care Coordination team.
    • Continuously evaluate and enhance training programs to ensure they meet evolving needs.
  3. Clinical Risk Management (10%)
    • Collaborate with the Care Coordination Director, Manager, and Senior Claims Risk Specialist to identify and address emerging clinical risks and trends.
    • Participate in high-risk claimant reviews and reconsideration meetings.
  4. Cross-Functional Collaboration (5%)
    • Represent Care Coordination on assigned projects and at conferences as requested by management.
    • Engage in cross-functional meetings to improve interdepartmental processes.

Qualifications:

  • Current unrestricted RN in state of residence, required
  • 2+ years of supervisory/leadership experience, required
  • 2+ years of experience in long term care or case management required, experience as a FLTCIP Care Coordinator preferred
