LICENSED SOCIAL WORKER

Position Location: Lincoln – Care Coordination Department

Hours: Full-time - 40 hours / week – Four 10-hour days or five 8-hour days

Primary duties:

· Assist in the overall management of HAN’s patients, particularly those with psycho-socio-economic needs

· Assist patients with transitions of care

· Connect with community resources

· Serve as a liaison between patients and other agencies

Qualifications:

· State of Maine LSW license required

· Must have excellent customer service skills

· Must be proficient with computers

· Prior care coordination experience preferred
